SYNOPSIS
During a Search and Rescue (SAR) mission in the vicinity of Hook Head, Co. Wexford on the afternoon of 6 December 2015, a Sikorsky S-92A helicopter crew located and winched two casualties from the water. During the winching operation one of the casualties, a 14 year old girl, slipped from the rescue strop and fell into the water from a height of approximately 45 ft. The casualty was recovered immediately from the water and winched up to the helicopter again. Both casualties were transferred to a nearby hospital in Waterford for medical treatment. The Investigation was subsequently informed by An Garda SÃochána that one of the casualties, a 14 year old girl, had died on 10 December 2015.

SYNOPSIS
While attempting to land on a canal bank adjacent to a restaurant/guesthouse in Abbeyshrule, Co. Longford, the main rotor blades of the helicopter made contact with a timber-clad wall of the building. This caused the tail of the helicopter to swing rapidly towards the wall and collide with it. The tail boom separated from the helicopter, broke apart, and came to rest in the canal. The helicopter rolled to the right and impacted with the canal bank. Debris was scattered over a large area of the canal and its bank. There was no fire. The Pilot and Passenger evacuated the helicopter and were uninjured. One person, who was in the building at the time of the event, received minor injuries.

SYNOPSIS
The light aircraft, with the Pilot and one passenger on board, was returning to Cregboy from Letterkenny airfield (EILT). During the landing, the Pilot reported that the aircraft’s rate of descent increased unexpectedly, resulting in a hard landing and bounce. The aircraft subsequently touched down again on its nose wheel.
The nose gear assembly fractured and the nose and propeller of the aircraft contacted the ground. There was extensive damage to the aircraft. There were no injuries.
